Ready to register? Set up a FinalForms account to complete the online student registration process.

Registration forms must be completed in FinalForms. FinalForms is mobile-friendly and can be used on a mobile device, tablet, laptop or desktop computer. You can save your progress so you do not have to complete the entire process in one sitting.

When enrolling a student in grades K-6, please use our District Street Roster to determine which of our five elementary school buildings your child will attend and if he/she is eligible for school transportation.

Grade Documentation: Current report card, transcript, or progress report

Special Education Paperwork: (if applicable) The current ETR/MFE and IEP for students with disabilities, or the current Section 504 Accommodation Plan, or Gifted Identification information should be presented at the time of registration, if applicable. An official request will be made to the previous school for these documents as appropriate; however, having the documents upfront facilitates a smoother transition.

Students need to be five years old on or before August 1 of the current school year to enroll in kindergarten.

Kindergarten students at HHCS attend school all day Monday through Friday.

Prior to starting, kindergarten students need to be screened. Screening will take place at the building your student will be attending and will last approximately two hours. Kindergarten screening appointments are scheduled using our online appointment scheduler. A link to the appointment scheduler will be sent to parent/guardian by email once the student's FinalForms enrollment is complete and all required documents have been received.

Child's Birth Certificate or Passport:  An image of the original (or certified copy) of the student's birth certificate must be uploaded.

Parent/Guardian Current Driver's License, State Issued ID, or Passport: An image must be uploaded for proof of identity.

Home Owner or Renter : ONE of the following documents must be provided, and dated within the past 30 days, in order to confirm your residency in the Huber Heights City School District: Current Mortgage Statement, Current and Signed Lease Agreement (including signature page with the property owner), Current AES Statement -electric (full page to show service and mailing address; no final bill or disconnect), Current CenterPoint Energy Statement - gas (full page to show service and mailing address; no final bill or disconnect), or a Signed Purchase Agreement with an Occupancy Date.  Statements from the water department, trash, or from a cellular service DO NOT meet proof of residency documentation requirements and will NOT be accepted.

Residing with Family/Friends : If the parent/guardian is residing with their student in the home of family or friends, refer to the HHCS Adult Shared Residency Checklist and Affidavit for additional requirement for registration. 

Custody Decree:  (if applicable) Complete, current legal documents signed by the judge or magistrate, must be presented at the time of registration to confirm custodial/residential parent. Notify registrar of any future revisions.

Guardianship:  (if applicable) Complete, current legal court documents signed by the judge or magistrate, must be presented at the time of registration.

Intent to Gain Custody: (if applicable) Complete, current legal documents, indicating the date the case will be heard, must be presented at the time of registration.
