Students attending school must comply with all federal, state, and local immunization and health examination requirements. A complete list of immunizations required for school attendance can be viewed here.

Evidence of immunization is required at the time of enrollment in school. If all immunizations are not completed, then written evidence that the student is in the process of receiving such immunization and that it will be completed within 14 days of entering school is required prior to enrollment. Students failing to be properly immunized may be excluded from school attendance.

A student may be exempted from one or more of the immunizations if the parent and physician (if applicable) complete a Parental Objection Form for Student Health Immunization and agree to comply with the condition under which the exemption is granted.

Staff take every precaution to avoid accident or injury to our students. However, accidents do happen on occasion. Most are minor and do not necessarily require notification. Injured students are taken to the school clinic if a nurse or clinic assistant is on duty. If the clinic is not staffed, the injured student is taken to the school office. The student's well-being is assessed at that time, and, if staff are concerned, parents/guardians are notified.

When a serious accident occurs (especially bumps or blows to the head), the principal and parents/guardians are notified immediately. Staff will follow the instructions listed on the student’s Health History and Medical Profile on Final Forms. 

Please ensure your Final Forms are up to date with contact information for both primary and secondary contacts so that your child's building can reach someone in the event of an accident, injury, or medical emergency.

Parents/guardians can call the school or use Final Forms to notify staff, clinic aides and district nurses of any new or existing medical conditions that may manifest themselves during school hours (i.e. food allergies, allergies, asthma, Epi-pen, seizures, diabetes, breathing treatments, other treatment, etc.). 

While medication administration is generally discouraged during school hours, our district recognizes that taking medication to treat certain medical conditions is sometimes a necessity. Any student who must take medication during school hours must abide by the following:

1. Print and have a parent/guardian and physician/prescriber complete and sign the Request for Medication Administration Form. The completed and signed form and medications to be administered must be submitted to the school’s main office before the start of the day on which the student must begin taking the medication.
2. Medications to be administered must be in the original prescription container, labeled with the student’s name, medication name, frequency of administration, dosage, and prescribing physician's name.
3. Medications brought to school must be secured (locked) and dispensed by an administrator, certified personnel, or designee.
4. Each dose administered will be recorded on the medication log with the date and initial of person dispensing that dose.
5. Students are responsible for reporting to the main office to receive their medication.

If parents/guardians feel their student's health or life is dependent on the immediate availability of the medication or the student has the capability and maturity to self-administer the medication safely, they can submit a Request for Student Possession or Self-Administration of Medication Form. 

Please remember to keep ill children home from school. It is important that students and staff that have the following symptoms remain at home and call a medical provider (cough, fever, headache, sore throat, vomiting/diarrhea, abdominal pain, loss of taste or smell, etc.). Students exhibiting these symptoms at school will be sent home. 

If experiencing illness during the school day, students will be given a clinic pass and sent directly to the clinic or office. Custodial parents/guardians will be notified if the student is too ill to remain at school. If the parent cannot be contacted, the relative or other contact listed under Health History and Medical Profile on the student's Final Forms will be contacted.
